WHEN TO COLLECT SPECIMEN
Urine specimen is collected between
4-5 PM (or 5 to 6 hours before bed),
just BEFORE late afternoon
supplements are taken.
MATERIALS IN KIT
1 Specimen time/date label
1 Urine collection cup
1 Urine transport tube
1 Pipette for transferring urine into
transport tube
1 Prepaid postage label
1 Patient information form
1 Resealable Ziploc bag
1 4x4 gauze pad to wrap specimen
tube
INSTRUCTIONS

HOW TO COLLECT SAMPLE
Begin to void and collect the
mid-stream sample of urine.
Transfer sample into transfer
tube with pipette provided.
Fill out time/date label and
apply to specimen tube.
Return specimen on the day of
collection or freeze and mail the
next day. *
Call FedEx to schedule a pick
up.

MAILING INSTRUCTIONS
Wrap sample in gauze pad
provided and place in
resealable bag.
Fill out patient information form.
Place specimen and form in
box provided.
Mark URINE on enclosed
mailing label.
